Antifungal
Hypersensitivity. Severe renal impairment (IV). Co-admin w/ astemizole, bepridil, cisapride, dofetilide, levacetylmethadol (levomethadyl), mizolastine, pimozide, quinidine, sertindole, terfenadine, atorvastatin, lovastatin, simvastatin, triazolam, oral midazolam, ergot alkaloids (e.g. ergotamine, dihydroergotamine, ergometrine, methylergometrine), eletriptan, nisoldipine, felodipine, disopyramide, methadone, ranolazine, eplerenone, dronedarone, halofantrine, irinotecan, lurasidone, and in patients w/ renal and hepatic impairment, colchicine.
Patient w/ risk factors for CHF (e.g. ischaemic and valvular disease, COPD, renal failure, oedematous disorders). Renal and hepatic impairment. Pregnancy and lactation. Patient Counselling May impair ability to drive or operate machinery. Monitoring Parameters Monitor liver function, serum concentrations, renal function, signs and symptoms of CHF.
Nausea, vomiting, diarrhoea, abdominal pain, anorexia, constipation, dyspepsis, dysphagia, flatulence, gastritis, taste perversion, ulcerative stomatitis, gingivitis, gastroenteritis, increased appetite, rash, pruritus, urticaria, angioedema, alopecia, toxic epidermal necrolysis, headache, dizziness, somnolence, decreased libido, insomnia, depression, tremor, anxiety, fatigue, malaise, asthenia, vertigo, HTN, hypokalaemia, peripheral oedema, mild transient increase in liver enzyme values, hepatitis, adrenal insufficiency, gynaecomastia, male breast pain, albuminuria, impotence, UTI, cystitis, abnormal renal function, menstrual disorder, transient or permanent hearing loss, fever, bursitis, myalgia, pain, herpes zoster infection, injury, rhinitis, upper resp tract infection, sinusitis, pharyngitis, neutropenia, face oedema, jaundice, hyperglycaemia, confusional state, renal impairment, dysphonia, erythematous rash, chest pain, chills, hypomagnesaemia, cough, hyperhidrosis. Rarely, anaphylaxis, Stevens-Johnson syndrome, neuropathy.
May increase the plasma concentrations of oral anticoagulants, digoxin, cilostazol, alprazolam, midazolam (IV), repaglinide, corticosteroids (e.g. budesonide, dexamethasone, fluticasone, methylprednisolone). May increase plasma concentration w/ HIV protease inhibitors (e.g. ritonavir, indinavir, saquinavir), erythromycin, clarithromycin. May reduce plasma concentration w/ isoniazid, carbamazepine, nevirapine, phenytoin, Phenobarbital, rifampicin, rifabutin. May reduce absorption w/ PPIs, antacids, antimuscarinics, histamine H2 receptor antagonists. Concomitant use w/ dihydropyridines may cause oedema. May increase negative inotropic effects of verapamil. May increase risk of potentially fatal resp depression w/ fentanyl.
